Pediatric Generalized Anxiety Disorder - Free Essay Example

Both children and adolescents occasionally experience the anxiety that accompanies stressful events. Alarmingly, it isnt uncommon that many children and teens in the US are diagnosed with an anxiety disorder. Anxiety disorders and co-occurring disorders like depression and separation anxiety disorder (SAD) have a dramatic effect on a childs quality of life. Generalized anxiety disorder is one of the most common types of anxiety disorders, and unfortunately, it affects a large portion of the pediatric population. Children with a generalized anxiety disorder (GAD) are described as having uncontrollable feelings of fear that are overwhelming and excessive in nature. These children persistently worry about before, during, and after a daily activity has happened. Generalized anxiety disorder is estimated to affect 15% of children in the US with an age of onset of 8.5 years. The exact cause of pediatric GAD is unclear and disputed among many professionals; however, its widely believed that there are both biological and environmental etiologies. For example, parents with pre-existing anxiety may transfer the same affliction to their children genetically. Alternatively, children who are being raised by overprotective parents attempting to reduce anxious behaviors are at-risk of manifesting GAD. Children and adolescents will report private events like physical pains that include: heart palpitations, muscle tension, stomach aches, and headaches. Some notable overt behavioral manifestations include insomnia, latching onto family members, and a severe lack of attention. Covert behaviors are marked by feelings of nervousness, fatigue, stress, and restlessness. Involuntary actions such as sweating, hyperventilation, increased heart rate, and trembling are commonly observed symptoms. Surprisingly, children with isolated GAD do not typically produce anxious behaviors when conversing, and they can maintain normal conversation with their peers. There are skills that are slightly impacted, but these issues are not significantly problematic when it comes to a childs communication repertoire. Children with GAD have problems with assertiveness, give minimal eye contact with partners, interject with fewer comments, and ask fewer questions during conversation. In addition, children with GAD have smaller groups of friends, yet they are still preferred over children with other anxiety disorders, especially those with social phobia (SP). Although GAD does not have a profound effect on communication by itself, social skills are greatly impaired when other disorders are present. For example, in children with ASD and comorbid GAD: the social use of language is weakened; thus, maintaining conversation is difficult. Retrieval of verbal cues is also disrupted because of increased anxiety which ultimately leads to communication breakdown. As for the behavioral dimension of GAD, we unfortunately see more problem behaviors associated with this disorder. Parents often report their children avoiding their responsibilities by complaining too much. Clinicians will find that these patients will worry excessively about themselves and their family members, and sometimes think a natural disaster will threaten their safety. A prominent avoidance behavior involves avoiding going to school because they worry about their performance in school and other extracurricular activities; these kids strive for perfection which results in low self-esteem. Despite having relatively small social difficulties, children and teens with GAD can have strained relationships with family and friends because they unrealistically seek constant reassurance from them. Finally, they may exhibit an aggressive temper or act downtrodden when they receive critique that appears to be harsh from teachers or parents. Treatment interventions for GAD can vary depending on a childs personality, their level of development, and familial background. Positive reinforcement would be the most effective therapy option for clinicians treating pediatric GAD. Depression, social anxiety disorder/social phobia (SP), and separation anxiety disorder (SAD) are common comorbid disorders which can have a detrimental effect on a childs self-esteem and self-image. Negative reinforcement could potentially have a negative effect on a childs well-being and may trigger an anxiety attack. This is especially true if the stimulus is something especially unpleasant like a buzzer or a siren, which may lead the child to believe they are in imminent danger. Clinicians should consider using positive reinforcers like candles (olfactory), music (auditory), and coloring books (tangible) that will allow the child to alleviate stress and provide a relaxing atmosphere while they work on their goal for the day. An intermittent reinforce ment schedule would be the most effective reinforcement schedule for children with GAD since these children are usually beyond the phase of acquisition; therefore, no continuous reinforcement schedule is necessary. A ratio schedule of reinforcement is the most suitable since it will lend no element of surprise to the child. The child knows theyll be reinforced after they accurately perform the correct number of responses; nevertheless, its best to start with a fixed schedule and then fade into a variable one. Starting with a variable schedule when first treating a child could increase a childs paranoia; thus, rendering therapy obsolete. A task analysis is another potential intervention for children with GAD. Anxiety is marked by increased stress, and these patients will need to learn how to cope with exasperating situations. Clinicians can perform an empirical task analysis by modeling activities like meditating, yoga, and exercise. A rational task analysis which involves asking a therapist is another option for treating GAD. Therapists may recommend combining cognitive-behavioral therapy (CBT) with medication most notably, selective serotonin reuptake inhibitors (SSRIs). Pediatric generalized anxiety disorder affects many teens and children in the United States, and it unfortunately goes under-recognized because many clinicians assume that worrying is a normative part of childhood development. Although, communication is not severely impaired, behavioral symptoms are significantly profound and recognizable. I will reflect on an incident, which occurred whilst I enrolled for my nursing training in college (McAllister, 2013). I will utilize the various aspects of reflective writing model to discuss my training experience and its future implications. I will focus on my knowledge, emotions and action to elucidate how an incident during my training prepared me for a career as a registered nurse. The incident- From the very beginning of my training, I had spent a great deal of time observing how the senior staff approached all patients. I had completed 8 months of my nursing training when this incident occurred. I consider it as an important and critical in-house incident which acted in the form of a wake-up call and created a challenging environment. Mr. Sly, aged 65 years had been admitted in the hospital after he sustained a fracture on twisting his left-ankle in a water puddle. On admission, he complained of severe pain due to fractures to the lateral malleolus on his left ankle. Pain is an unpleasant emotional and sensory experience caused due to tissue damage (Williams Craig, 2016). The accident resulted in undue anxiety to him and his family. A doctor treated his fracture with plaster-cast immobilization and assigned a nurse to take care of him. However, the nurse in-charge did not make any effort to look after his needs or talk to him. He was distressed and had to be calmed down by his family members. My feelings- During our training sessions, I noticed that the nurse-in-charge did not attend him, nor did she keep a record his medical notes. This incident was upsetting, as a staff was responsible for his distress. I could feel his pain and agony. I was anxious and nervous at the same time as I wanted to help him but could not do so without assurance from a senior nurse. There was swelling and bruising in his ankle. Based on my training knowledge, I felt the need of talking to him and recommending my senior to increase his drug doses. Evaluation- I approached him and introduced myself to build a good rapport. I knew what was to be done but needed a reassurance from a senior registered nurse. I tried to initiate contacts with the senior RN who could provide him necessary resources for relief. I assured him I will be back and went to the senior to talk about his condition and how to relieve his pain. My training academics helped to understand the pathophysiology of his twisted ankle. I approached the senior for a reassessment and change of medicine dose. I took his medical charts along and showed that he was on a five hourly administration of 10mg morphine. Morphine is the most effective opioid to reduce acute pain. My mentor analyzed his charts and we changed the dose to five hourly administration of 20 mg morphine (Macintyre Schug, 2014). A competent and responsible nurse was advised to look after him following the declining unprofessionalism of the former nurse-in-charge. Explanation of the situation- We analyzed his medical charts because these records are the best way to monitor a patients conditions and needs (Abell, 2015). On analysis, she confirmed my thought of increasing the analgesic dose to reduce his pain (Macintyre Schug, 2014). On getting back to him, I obtained his informed consent for the reassessment and told him there might be a need to increase his drug dosage to reduce his pain. This seemed to calm him a little. I had the theoretical knowledge that lateral malleolus occurs due to fractures at the end of fibula bone and morphine is the most commonly used opioid to treat acute pain (Odak et al., 2016). It works on the central nervous system. We also prescribed some antiematics to reduce his nausea. He reported good spirit and better health conditions after the reassessment. I worked hand-in-hand with my mentor and observed how she administered the drug and monitored the changes observed in the patient. Understanding- I felt that the unprofessional attitude of the nurse-in-charge hinders care-giving practices. A nurse should always be empathetic towards patients. After a change in dosage of medicines, he did not complain of acute pain and we discharged him the day after. He thanked me for being a patient listener and communicating his problems to the senior. I realized that timely intervention of my mentor helped to reduce his discomfort (Bridges et al., 2017). Instead of being moved by pity for the patients condition, I should always seek guidance from a senior. Intervention form a senior always leads to a quicker outcome. I learned that a patients medical record is a fundamental aspect of nursing practice. Accurate documentation minimizes chances of errors and provides evidence for increase in drug doses when required. I also learned that pain is often related to subjective experiences (Wilcox et al., 2015). My future practice would include exhaustive study on pain management and patient records documentation. I will reflect on the ways of increasing effectiveness of nursing practice. I will prioritize my patients, show empathy towards them and try to make them feel comfortable. I will also focus on the use different assessment tools and VDS (verbal-description scales) to assess pain intensity and response to opioids (Athlin, Carlsson Gunningberg, 2015). I will also sharpen my communication skills because effective communication with a patient is a prerequisite for building a strong patient-nurse relationship. Therefore, I conclude from the incident mentioned above that, my nursing training helped me gain knowledge on the different aspects of nursing practice and providing care and relief to patients. Planning- Planning is a very important aspect achieved while exploring via thorough consideration of: task and objective, resources, alternatives and brainstorming. This helps in reaching a decision and reviewing it with all the participants. The plan is then evaluated and executed. Principle Centred- Principle centred leaders are strongly affected by ethical and moral principles. Their quality of work shows this used of ethics which involves honesty, fairness, trust, justice and equity among others. POWER AND INFLUENCE: With leadership comes power and influence which are important factors that help in the motivation of team. Power is the ability to exert influence that is, to change the attitudes or behaviour of individuals or groups. Influence is any action or examples of behaviour that causes a change in attitude or behaviour of another person/ group. (Leadership- Lecture Notes, Pg. 37, 2012) LEADERSHIP THEORIES: Major leadership theories are mentioned as under: Trait Theories- Helps in identification of personal characteristics of leaders which differentiate them from non-leaders. Some of these characteristics include energy, intelligence, and resourcefulness etc. Style Theories- Related to behavioural approach as leaders are born not made. Shows concern for people and task. Can be authoritative or democratic. Contingency Theories- Such theories focus on particular variables that determine which style is best suited for any situation. As per this theory not every style is suitable for all situations. Success depends on leader-member relations, degree of structure in the task and power/authority of the position. Participatory Theories- Suggests that the ideal leader is one that takes the input of others into account. These leaders encourage participation and contributions from group members and make them feel more involved in the process. It calculates to what extent the opinions of members are utilised in decision making. LEADERSHIP vs. MANAGEMENT Often people confuse leadership with management though they both differ from each other on so many levels. A manager is appointed while leadership is earned in most cases. Managers can be characterised as people who imitate, establish clear targets, make short term decisions, solve short term problems, enact visions and do things right. They employ the so-called hard skills such as planning, directing, organising and keeping score. On the other hand, leaders employ many of the softer skills. They direct and guide people; influence thoughts and behaviours; motivate; encourage work towards goals; take risks; innovate; have a long-range perspective; have their eye on the horizon; create visions and do the right things. (Leadership vs. Management, Leadership in the Construction Industry, Pg. 5) According to Covey (1992): Leadership deals with direction- with making sure that the ladder is leaning against the right wall. Management deals with speedà ¢Ã¢â€š ¬Ã‚ ¦. Leadership deals with vision-with keeping the mission in sight- and with effectiveness and results. Management deals establishing structures and systems to get those results. Leadership focuses on the top line and management on the bottom line. Leadership derives its powers from values and correct principles. Management organises resources to serve selected objective to produce bottom line. Management and Leadership are not mutually exclusive; in factà ¢Ã¢â€š ¬Ã‚ ¦. leadership is the highest component of management. To summarise, the manager must have certain leadership qualities to take their organisation towards success by motivating people to achieve that success. According to the Eagan Report, The Eagan Report identifies five drivers of change necessary to ensure greater profitability as well as better quality in the Construction Industry: Committed Leadership A focus on the customer Integrated processes and teams A quality driven agenda Commitment to people (Leadership- Lecture Notes, Pg. 40, 2012) LEADERSHIP IN CONSTRUCTION INDUSTRY: Construction industry is very complex and is often met with various problems which are non-completion of a job, non-availability of labour, Community barriers between workers and goods availability which leads to delay in productivity and effectiveness on construction sites many of which arrive from inadequate understanding of leadership and job relationships. Leadership in different sectors of construction industry: Health and Safety- Leadership in health and safety sector is of strong importance as this sector deals with health related issues for labour along with other people who are related to a construction project. This sector has zero tolerance policies, industry initiatives and high level of investment. Sustainability- Sustainability is an important sector for the industry to consider. The construction done on a site should be sustainable and environment friendly in current time as per the urgency of the sustainability agenda. So the leader should be well aware of the ill effects a particular action that is performed will have on the nature. Business ethics- Business ethics are also a major factor in the success of a task. A leader must be well aware of all the business rules and changes that are occurring in the construction industry to make his team lead in the right direction and as per the building regulations. This provides a strong base for a leader by making them flexible and well aware of the current surroundings. Education and Training- Proper education and training helps in becoming an ideal leader. This sector focuses on education and experience of the leader and group members to put alike people with each other. This helps in creating harmony and trust between the members which in turn helps in successful completion of a job. In construction industry the requirements for an effective leader are as follows: Good communication skills Understanding of business Strategic vision Charisma and Inspiration Integrity Good listener Flexible i.e. open to changes Experience The weakest leadership qualities are identified as poor communication skills, lack of time, lack of general management skills, impatience, lack of decisiveness and not delegating enough. Also there is a presence of untapped leadership potential within the industry. Many areas of weak leadership can be addressed through industry training and other leadership development programmes. Improvement in communication and general management skills are two areas in particular that are of topmost priority. Organisational culture and lack of opportunities are the major drawbacks in the construction industry which stops an individual from reaching their highest potential. How to improve leaders? As discussed above many of the problems in construction industry arise due to poor leadership skills and to improve leadership qualities following steps should be taken: New experiences, the most valuable way to improve leadership ability. This includes mentoring or anything else that gives an individual a new hands-on experience and exposure. Clear direction and guidelines are important to improve individual leaders in the industry. This reinforces the importance of empowerment, development of leaders at all levels of seniority and corporate strategic planning. Skills, education, training and qualifications are also required to be present in any leader. To achieve the above qualities various character development workshops and programmes can be held within an organisation to make people understand the concept of leadership better and to know more about leadership skills tailored specifically for construction based professionals. These workshops must include programmes focusing on communication skills and general management training, education and training, mentoring schemes, work experience, and secondment programmes. They should also include strategies for identifying the leaders of the future, and for rewarding and celebrating success. CONCLUSION: Leaders are of utmost requirement these days to complete a given job. The leaders help in creating group cohesiveness which helps in proper utilisation of the teamwork. The leaders are born and not created though certain steps can be taken to brush up their skills. The leadership development workshops can help in improving the leadership skills of people which will in turn help in the betterment of the construction industry. Also, the main drawbacks in the construction industry related to incompletion of a task can be tackled effectively if positive steps are taken in that direction. Great leaders will be created that will work for the industry and give their hundred per cent to achieve a task.
